MCoC is providing the attached forms to give New and Renewal Project Applicants the opportunity to provide additional information that may not be covered by the esnaps application.
Use of these Supplemental forms is TOTALLY OPTIONAL – you are not required to complete or submit these forms, but doing so might provide the Selection Committee with information that would allow for partial points in areas where an application would otherwise not receive any.
There are two forms – one for New Projects, and one for Renewal Projects. Not all questions are relevant to all project types. Please be sure to clearly identify which project you are completing the form for, and if you have multiple applications, please complete a separate form for each project you wish to provide information for.
